1. Offers are valid at Curewell all Store & associate wellness Centers Only.
2. Visit www.curewell.co.in / Privilege Wellness card to view the Service network & offers.
3. Card holders are requested to present their I’D details at Our health care Center to avail benefits, if they are at offline visit.
4. In patient discount are not applicable for patients availing insurance & other health benefit cards.
5. Offer in the card cannot be clubbed will any other on going promotions or conditions.
6. Privilege Card, reserves the right to amend the privileges as well as terms and conditions of its sole discretion without any prior notice.
7. The offer applicable to this card is valid for life time but it need to renewal or to be eligible for auto renewal after One year.
8. The user acknowledges that Curewell EasyLive is merely offering the card facility for enhanced privileges at healthcare facilities of Privilege Wellness Card & is not involved in their debiting or crediting of any money or any monetary value from the card of the user.

CARD ACTIVATION STEPS:

You will be required to send an email to cards.support@curewell.co.in or hard copies with the following information.

a.fill the Personal assessment of Card Holder & all dependents
b. mobile number in use
c. Email ID in use
d. self attested scanned copy of adhar card
e. family member(dependents) adhar card
f. one photo(pass port size) of card holder or Family Group photo
